We conduct business under our corporate name NorEast Franchise Group, LLC and under the A Place At Home trade name. Our business is operating the A Place At Home Business franchise system and granting franchises to third parties like you to deyelop and operate an A Place At Home Business. We haye been offering franchises since 2017.

A Place At Home,' LLC's principal business address is 1 1308 Davenport St. Omaha, Nebraska 68154 from which A Place At Home, LLC utilizes the Marks and System in connection with the operation of A Place At Home business in Omaha, Nebraska. A Place At Home, LLC commenced the operations it's A Place At Home business in August, 2012. You will not conduct business directly with A Place At Home, LLC. A Place At Home, LLC has not in the past and does not now offer franchises in any lines of business. A Place At Home IP, LLC Our affdiate A Place At Home IP, LLC is a Nebraska limited liability company established in April 13, 2018. A Place At Home IP, LLC's principal business address is 11308 Davenport St. Omaha, Nebraska 68154. A Place At Home IP, LLC was established for the purposes of owning and licensing the Licensed Marks.

Business Offered

We have developed and presently license a system (the “System”') for the operation of a business that provides non-medical in-home care services, senior living placement services, care coordination and health care organization staffing services (the “System Services and Products”). The System is presently identified by the A Place At Home trademark, the A Place At Home logo and such other trade-names, trademarks, service-marks, logotypes, and commercial symbols as we may designate, modify and adopt from time to time for use in the System and as same may or may not be registered with the United States Patent and Trademark Office (collectively referred to as the “Licensed Marks”').

Initial Fees

When you sign a Franchise Agreement you must pay us an Initial Franchise Fee of $33,000. The Initial Franchise Fee is based on a territory (an “Operating Territory”') that contains a minimum of 30,000 Qualified Residents (a “Base Operating Territorv”~). If the Operating Territory that you select and we agree to award contains more than 30,000 Qualified Residents then your Initial Franchise Fee will be increased

Financing

We do not offer direct or indirect financing. We do not guarantee any note, lease or other obligation on your behalf

Franchisee Revenue and Profit

The FTC's Franchise Rule permits a franchisor to provide infonuation about the actual or potential financial performance of its franchised and/or franchisor-owned outlets, if there is a reasonable basis for the infonuation, and if the information is included in the disclosure document. Financial performance information that differs from that included in Item 19 may be given only if (1) a franchisor provides the actual records of an existing outlet you are considering buying; or (2) franchisor supplements the infonuation provided in this Item 19, for example, by providing information about possible performance at a particular location or under paiticular circumstances.
