Physique 57 Franchising LLC (“we”, “our” or “us”) is a New York limited liability company that was formed on May 10, 2018 and has its principal place of business at 24 West 57th Street, Suite 805, New York, New York 10019. We do business under our corporate name and under the franchise name “Physique 57”. We will refer to the person who buys this franchise as “you” throughout this Disclosure Document. If the franchise purchaser is a business entity, “you” also includes each partner, shareholder and/or other owner of that entity. We have never offered franchises in any other line of business. We have offered franchises as of December 20, 2018 the date this Disclosure Document was registered in the State of New York. Our agents for service of process are listed in Exhibit A.
