Healthy Fast Food, Inc. was incorporated in Nevada on November 14, 2005, and the name of the corporation was changed to U Swirl, Inc. on May 16, 2011. The principal business address of USI is 265 Turner Drive, Durango, Colorado 81303 USA. The principal business address of U Swirl, Inc. is the same as USI’s address. USI’s agents for service of process are disclosed in Exhibit A.
