We have no parents or predecessors required to be disclosed in this Item. Our affiliates (hereinafter, collectively “Affiliate" or “Affiliates") are: (i) FacialMania LLG, a Florida limited liability company with offices at 16950 Jog Road, Suite 111, Delray Beach, Florida 33446, which has operated a Facial Mania Med Spa like those offered hereunder since 2017; '(if). Lasers Technology USA LLG, a Florida limited liability coiripany with offices: at 7888. La Mirada Drive, Boca Raton, Florida 33433; and (iiij FagialMania IM LLG, a Florida limited liability company with offices at 601 Gongress Ave,, Ste. 409, Delray Beach, Florida 33445, which has operated a Facial Mania Med SpA like those offered hereunder since 2018; (iy) Gollagen Drops LLG, a Florida limited liability company with offices at 7888 La Mirada Drive, Boca Raton, Florida 33433. Our Affiliates have never offered franchises of any sort. Of our Affiliates, only Fac^ialMaNia LLG and FacialManIa IM LLG are Facial Mania franchisees, they have converted their existing Facial Majmia Med Spas to franchised Facial Mania outlets in July 2019.

The total initial investment necessary to begin operation of a Facial MANiAjfanchise is estimated to range from $250,300.00 to $688,500.00, the total amount of Item 7. This includes an initial franchisee fee in the amount of $39,000.00, the total amount of Item 5, Which must be paid tO the franchisor; Veterans and business owners underthe age of 30 receive a $4,000 discount on the initial fee. Facial Mania offers two fypes offranchises: (i) single unit; and (ii) multi^unit. Ifyou want development rights for a multi^unit franchise, you generally must pay us a development fee of $14,500, plus an initial franchisee fee in the amount Of $29,000 for each additional Facial Mania Med Spa, but we will credit the development.fee against the initial franchise foe when you sign the franchise agreement for each, additional unit to be developed ifyou are not then in any default, (see Item 5 and Item 7 for more:detailed information).
